A Guardian Conservator will carry a caseload of 35-42 individuals.
Advocate for individuals for least restrictive life choices that enable them to live full and abundant livesWork with individuals and families in the development and service delivery of support decision-makingWork closely with courts, including annual reportsSupervise and monitor client personal and health careThis can include authorizing medical, therapy, treatment, support, vocational, and other servicesSign legal documents such as consents, admissions, and lease agreementsWork as a member of the support team to ensure appropriate living environment and servicesTravel to visit clients.
Provide substitute decision-making regarding medical emergencies and other sudden changes in clients' situationsBe responsible for the financial management and oversight of individuals assigned to themWork with financial team to oversee financial assetsBe a member of an on-call schedule for client emergencies
Knowledge of Alzheimers care, dementia care, memory care, autism support or hospice care is preferred.
Strong observational skills for patient monitoring and recognizing changes in health status.
Excellent communication skills for collaborating with healthcare teams and supporting residents social needs.

Qualifications:
Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care is highly desirable.Knowledge of Alzheimers care, dementia care, memory care, autism support or hospice care is preferred.
