We are seeking an InformationReferral Specialist to provide information to our consumers in response to service and eligibility inquiries.
Job Duties/Responsibilities:
Perform initial and ongoing assessments of the health and functional status of members Develop community-based care plans and related service packages Participate in the development of a member's Individual Care Plan (ICP) Arrange and coordinate the provision of appropriate community long term care, social support services or other community-based services Coordinate direct services to Members Monitor the provision and effectiveness of community services Track member transfers from one setting to another and work with the PCT to adjust the ICP as appropriate Promote the independent functioning of members and provide services in the most appropriate, least restrictive environment Participate in the determinations of a member's need for institutional and community long term care services Maintain records for each member in accordance with regulations and standards
Required Knowledge/Skills/Abilities:
Considerable knowledge of the principles and practices of social work Working knowledge of the social, emotional, and physical processes of aging Ability to gain and hold the confidence of clients and their families Ability to obtain information of a personal nature through interviews Draw appropriate conclusions and make recommendations Ability to establish and maintain harmonious working relationships with consumers and staff Knowledge and skill necessary to work successfully within an interdisciplinary team; ability to express clearly, both orally and in writing
Education/Experience :
Bachelor's degree in Social Work or related field preferred Minimum of two years' professional experience in the care of persons 65 and older, with at least one year involving work in a setting involving health care services Current Social Work license (LSW or higher) preferred What you'll get: The ability to work with an enthusiastic team of like-minded individuals.
