Optimus Health Care—the largest provider of primary health care services in Fairfield County—is looking for a dedicated clinician to join our team! With multiple locations in Bridgeport, Stratford, and Stamford, our mission is to be a lifelong health care partner, dedicated to achieving optimal wellness for the communities we serve. Optimus Healthcare is looking for a full-time experienced LCSW to join our team based model of care delivered in a kind and compassionate manner. The Clinician should be comfortable working with ages 5 and up and a working knowledge of Spanish is preferred. This currently is a hybrid role with three days onsite and two days remote. The LCSW provides direct services to Optimus' patients and families with the goal of developing patient independence and self-sufficiency, when applicable. Also provides treatment for patients who are diagnosed with a mental illness or refer to appropriate service provider. Oversees, administers, coordinates and/or assists in other designated Optimus programs and functions as assigned.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES
Provides clinical behavioral health services and some case management, information & referral services to individuals and families. This includes psychosocial assessments and psychotherapy, treatment planning, collaboration with and referral to other members of the health care team internal and external. Maintains appropriate documentation. Generates initial treatment plans and updates every three (3) months. Discharge summaries are generated thirty (30) days after patient transferred, discharged or patient terminated treatment. Responsible for department operation during Director's absence. Representation of the Social Services Department and / or OHC as a liaison at agency and community meetings. May delegate some of this responsibility to other staff. Assist in program development as directed by the Director of Behavioral Health and / or Administration. Responsible for the accurate and complete preparation of encounter information including appropriate coding for diagnosis and procedure; DSM-5 and CPT Coding to ensure prompt billing for services rendered.
EDUCATION
Master's degree in social work from an accredited university
EXPERIENCE
A minimum of two (2) years of related experience. Experience preferred in the areas of culturally diverse populations. Combination of clinical, crisis intervention, systems, case management, conflict-resolution, behavior modification, program development, child development, state and local resources, and group skills needed. Good verbal and written communication skills are required. Basic computer skills desired. Familiarity with tele-medicine and prior experience a plus
LANGUAGE SKILLS
Working knowledge of Spanish is preferred
LICENSURE / CERTIFICATION
Licensed under the State of Connecticut Department of Public Health. A valid driver's license is also required. Working for
