Monday-Friday, 8:00 AM-5:00

PM EST Work Arrangement:

Hybrid (1-4 days per week onsite, field visits, and remote work)

Contingent/Temporary (Potential Temp-to-Hire) About the Role The Care Manager is responsible for managing and coordinating non-clinical services for members, including children and youth populations, in accordance with state regulations and contractual requirements. This position supports members through assessments, care planning, resource coordination, and collaboration with caregivers, families, community partners, and healthcare providers. This role is member-facing and requires regular interaction in community settings, member homes, Department of Social Services (DSS) offices, and other designated locations. Key Responsibilities Manage and coordinate member service plans within the scope of the role and applicable regulations. Conduct telephonic and face-to-face assessments using standardized tools and predefined questionnaires. Identify members who may benefit from additional services and coordinate support with clinical care management teams. Assist with the coordination and management of non-clinical needs for members with chronic conditions, disabilities, and complex social needs. Develop short-term and long-term goals in collaboration with members, caregivers, families, support systems, and providers. Facilitate access to community resources and support services. Maintain accurate documentation and follow established care management processes. Travel throughout assigned counties to meet with members, community partners, and DSS staff as needed. Required Qualifications Bachelor's degree required. Minimum of 1 year of experience working directly with children, youth, families, child welfare populations, or related community-based organizations. Equivalent combination of education and relevant experience may be considered in accordance with state and contract requirements. Ability to conduct face-to-face member visits and travel throughout assigned service areas. Strong communication, organization, and care coordination skills.
