The Department of Pediatrics, Division of Adolescent & Young Adult Medicine is seeking a full time Substance Use & Behavioral Health Linkage. Under the direct supervision of the Co-Principal Investigators and the Sr. Research Program Coordinator, the Community Linkages Specialist will assist in the day-to-day logistical and operational tasks involving the provision of connections and linkages to community services for adolescents aged 12-17. Specific Duties & Responsibilities Work with patients with substance use and other complex social and behavioral health conditions to identify and connect patients and families with appropriate resources.
Provide linkages to resources, support, outreach and coordinated care to patients.
Assist with interdisciplinary team crisis intervention, problem solving and idea sharing.
Facilitate enrollment and appointments for specialty substance use services and behavioral health care.
Manage tracking and utilization of resources.
Register and schedule patient appointments and referrals when needed.
Access and interpret patient coverage data in EPIC.
Screen patients for eligibility and assist those who are applying for assistance programs.
Maintain strong working relationships with members of Johns Hopkins Medicine and other organizations for needed assistance.
Maintain expertise on various benefit resources that may aid patients in gaining access to substance use and other behavioral health service delivery systems.
Schedule appointments to meet with patients and follow up as necessary.
Interpret and obtain understanding of patient coverage information.
Receive patient referrals from the JHH health professional staff and interfaces regularly with medical providers and support staff.
Participate in team meetings for the purpose of improving services to patients and maximizing patient benefits.
Identify problems and make improvements.
Demonstrate cultural competence and conflict resolution skills.
Work with agencies, providers and clinical staff to trouble shoot problems in communication regarding substance use and other specialty care programs.
Assist patients with bus or other transportation needs/arrangements per guidelines.
Maintain patient confidentiality.
Assist with clinical administration duties such as patient check out and scheduling follow-up appointments.

Experience with provision of adolescent group services is a benefit

Ability to communicate effectively across disciplines and within a variety of cultures. Minimum Qualifications High school diploma or graduation equivalent.
Two years of related experience.
Additional education may substitute for required experience and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula. Preferred Qualifications Fluent in Spanish and English highly preferred.
Training in Social Work, Behavioral Health or Addiction Services highly preferred
Proficient with databases and spreadsheets
