Job Description
Social Services Coordinator (LMSW Required) Heritage Post Acute is seeking a compassionate and dedicated Social Services Coordinator to join our interdisciplinary team. The ideal candidate is committed to enhancing the quality of life for our residents through exceptional psychosocial care, advocacy, and discharge planning. Position Summary The Social Services Coordinator is responsible for assessing residents' psychosocial needs, coordinating care and discharge planning, providing emotional support to residents and families, and ensuring compliance with all federal, state, and facility regulations for long-term care. Essential Responsibilities Conduct comprehensive psychosocial assessments for residents. Develop, implement, and update individualized social service care plans. Coordinate discharge planning and transitions of care. Provide counseling, support, and crisis intervention to residents and families. Connect residents and families with community resources and support services. Advocate for residents' rights and promote resident-centered care. Participate in interdisciplinary care plan meetings. Maintain accurate and timely documentation in the electronic medical record. Ensure compliance with CMS, state, and facility regulations. Collaborate with nursing, therapy, dietary, activities, and other departments to meet residents' psychosocial needs. Participate in Quality Assurance and Performance Improvement (QAPI) activities.
