Job Overview We are seeking a compassionate and dedicated Hospice Licensed Master Social Worker (LMSW) to join our interdisciplinary healthcare team. In this vital role, you will provide comprehensive psychosocial support, counseling, and case management services to patients and their families facing end-of-life challenges. Your expertise will help ensure that patients receive holistic care aligned with their wishes, cultural backgrounds, and medical needs. Duties Conduct thorough patient assessments to identify psychosocial needs, emotional concerns, and family dynamics related to hospice care. Provide individual, family, and group counseling sessions utilizing evidence-based practices such as cognitive behavioral therapy, motivational interviewing, and clinical counseling techniques. Coordinate community resources to support patients' social, emotional, and practical needs, including housing, transportation, and financial assistance. Collaborate closely with medical teams to develop and implement personalized discharge planning and care plans that respect patient preferences and clinical guidelines. Maintain accurate medical documentation in electronic medical record (EMR) systems while adhering to HIPAA regulations and social work practice standards. Offer crisis intervention services when urgent emotional or psychological issues arise for patients or family members. Engage in ongoing clinical research initiatives related to hospice & palliative medicine to enhance care quality and patient outcomes. Qualifications Master's degree in social work (MSW) from an accredited institution; valid LMSW licensure required. Proven experience working with geriatric patients in hospice, palliative care, or acute care settings; experience with dementia care, Alzheimer's disease, PTSD care, or autism spectrum disorders is highly valued. Strong knowledge of hospice care principles, end-of-life care practices, medical terminology, CPT coding, ICD coding systems (ICD-9/10), and Medicare regulations. Demonstrated ability in patient assessment, case management, crisis management, and behavioral health interventions such as behavioral therapy and assisted living experience. Familiarity with EMR systems for documentation; proficiency in medical records management is preferred. Excellent communication skills for client interaction, interdisciplinary collaboration, and community resource coordination across diverse cultural backgrounds. Knowledge of HIPAA compliance standards along with clinical ethical guidelines relevant to social work practice in healthcare environments. Additional skills such as working with children or individuals with disabilities, experience in outpatient or inpatient settings, or expertise in CPT coding are advantageous. $75,000.00 - $80,000.00 per year
