Job Description

The Helen Ross McNabb Center, a prestigious provider of behavioral health services, is seeking a dedicated Case Manager / Counselor to join our team in East Tennessee. This role is pivotal in delivering compassionate and comprehensive mental health and recovery services to our clients. The successful candidate will facilitate client growth and wellness through direct counseling services, care coordination, and advocacy, aligning with our mission to improve the lives of the people we serve. Responsibilities Conduct comprehensive assessments to evaluate the psychological, emotional, and behavioral needs of clients. Develop and implement personalized treatment plans based on client needs and goals. Provide direct counseling services to individuals, groups, and families to foster resilience and support recovery. Coordinate care with other professionals to ensure a holistic approach to client wellness, including liaising with healthcare providers, social workers, and community resources. Monitor and document client progress, adjusting treatment plans as necessary to achieve optimal outcomes. Advocate for clients, enhancing their access to necessary community resources and services. Participate in multidisciplinary team meetings to discuss and optimize client care plans. Conduct crisis interventions as needed and provide ongoing support to manage emergencies effectively. Required Skills Strong interpersonal and communication skills Proficiency in clinical assessment tools and therapies Ability to manage time effectively and multitask with high attention to detail Knowledge of local resources and systems to support client care Capacity to work independently and as part of a team Crisis management skills
