Job Description

General Function:
The AHS Assertive Community Treatment (ACT) team provides non-traditional community-based mental health treatment to promote recovery and community integration to individuals who may be experiencing serious mental illnesses, substance use disorders, and other psychosocial stressors. The ACT Case Manager will be expected to provide services and supports outside of a traditional office environment and engage with the community to develop relationships and job opportunities for ACT consumers. Qualifications;
Knowledge and Training:
Bachelor s degree or higher in a field related to behavioral health or vocational services and at least one year of experience working with individuals with mental illness and or substance use disorder. Previous experience working with an ACT team or providing job development and employment services/resources is preferred. This individual will be part of a multi-disciplinary team with a person-centered approach and will require collaboration and consultation with team members of various roles as well as ability to maintain positive relationships community stakeholders.
Duties and Responsibilities:
Provide assessment, linking and coordination of community services within ACT fidelity Provide outreach services to consumers in the community, including homeless outreach services. Maintain up-to-date knowledge on all relevant laws, policies, practices, and services affecting consumers Be willing to work on a flexible schedule to meet consumer needs Maintain all program, agency, and consumer documentation up to agency and funder standards Develop and maintain resources and relationships for services including housing, transportation, etc. Work according to fidelity with the ACT model, including evidence-based practices and theories pertaining to ACT, such as motivational interviewing, etc. Maintain positive and collaborative community relationships Must be willing to provide transportation in personal vehicle Other duties as assigned EOE M/V/F/H/ "Promoting Wellness in the Community"
