Licensed Vocational Nurse -

Care Coordinator Qualifications:

Graduate of an accredited school of vocational nursing Current Texas State license as a Licensed Vocational Nurse One year experience as a Licensed Vocational Nurse in a clinical care setting. 2 years of home health experience (excluding private duty and hospice) Experience with orders, labs, and assisting the RN COM (case managers) Signature Health Services is a highly rated (rated at 4.5 star Home Healthcare provider per Medicare) and for over 30 years we have provided premier home health care to home-bound patients living within a 70-mile radius of the Greater Houston area. Skills:

Nursing skills as defined as generally accepted standards of practice. Must have good interpersonal and organizational skills.

Transportation:

Reliable transportation. Valid Texas driver license and valid auto insurance that means the State of Texas requirements.
