Job Description

Performs minimally complex social service work. Reviews Texas Medicaid & Healthcare Partnership (TMHP) internet portal for new and pending assessments. Responsible for conducting screening and evaluation of person(s) seeking admission into a Medicaid-certified nursing facility. Conducts face-to-face evaluation of said individuals to assess need for specialized services, alternate placement options for the individual, and the need for care in a nursing facility or other setting, such as an intermediate care facility for individuals with an intellectual disability or related condition(s). Works under moderate supervision with latitude for the use of initiative and independent judgment. Serves as the point of contact for individuals (or LAR) seeking IDD services. Functions as a liaison between clients (or LAR), internal/external providers, State Mental Health Facilities (SMHF), State Supported Living Centers (SSLC), and Nursing Facilities. Ensures the coordination of services to the persons served, particularly between Nursing Facilities and services within the DADS and DSHS system, to include joint discharge planning, participation in treatment plan reviews and revisions during the transition phase. Maintains documentation and assesses, evaluates and refers clients to needed services.
