As a Lead PT, you will play a pivotal role in guiding clinical practices, supervising PT staff, and delivering exceptional patient care. Your expertise will help shape comprehensive care plans, facilitate discharge planning, and ensure the highest standards of services in the outpatient setting. Duties Develop and oversee individualized care plans for patients of all ages, ensuring they meet clinical goals and adhere to best practices. Supervise and mentor PT staff, providing guidance on treatment techniques and PT-based interventions. Conduct comprehensive assessments utilizing knowledge of anatomy and physiology to inform effective treatment strategies. Manage discharge planning processes by collaborating with multidisciplinary teams to ensure seamless patient transitions. Utilize electronic medical record (EMR) systems efficiently for documentation, progress tracking, and reporting. Educate patients and families on their conditions and self-management techniques to promote recovery and long-term wellness. Stay current with industry standards by integrating evidence-based practice. Time management skills. Proactive work ethic. Requirements Solid knowledge of medical terminology, anatomy, physiology, and treatment modalities relevant to diverse patient populations. Experience working with EMR systems for accurate documentation and patient management. Ability to lead effectively while fostering a collaborative environment focused on patient-centered care. Strong communication skills for liaising with multidisciplinary teams and educating patients/families about their treatments plans.
