Care Coordinator |
Provo, UT Pay:
$21-24 / hour Overview We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring our home health patients receive the highest level of care and support throughout their healthcare journey. This position requires a strong understanding of medical terminology and administrative processes. The Care Coordinator will work closely with healthcare providers, patients, and their families to facilitate communication and coordinate care effectively. Duties . Coordinate patient appointments, ensuring timely scheduling and follow-up. Maintain accurate patient records, including medical history and treatment plans. Collaborate with healthcare providers to ensure continuity of care and effective communication between all parties involved. Assist patients in understanding their treatment options and navigating the healthcare system. Utilize medical terminology to accurately document patient interactions and care plans. Support administrative tasks within the office, including filing, data entry, and managing correspondence. Ensure compliance with all relevant regulations and standards in a medical or dental office setting. Requirements Previous experience in medical administrative support is preferred. (CNA, RCC, or any medical background.) Familiarity with medical terminology is essential for effective communication within the healthcare environment. Strong organizational skills with the ability to manage multiple tasks simultaneously. Excellent interpersonal skills to interact compassionately with patients and their families. Proficiency in office software applications for documentation and scheduling purposes. Experience working in home health is a plus Ability to work collaboratively within a team-oriented environment while maintaining professionalism.
