As a Referral Coordinator at Fourth Street Clinic, you'll play a crucial role in connecting patients to specialized services and ensuring they receive the care they need. You'll collaborate closely with medical providers, behavioral health staff, and community partners to coordinate both internal and external referrals, schedule appointments, and facilitate follow-up care. Your day-to-day responsibilities will include managing incoming phone calls from patients and clinics, verifying insurance eligibility, and guiding uninsured patients to community resources. You'll also oversee various voucher programs, such as the IHC Diagnostic Service Voucher and HCI Colonoscopy Voucher, while providing trauma-informed, patient-centered support. Engaging with patients, staff, and community health centers, you'll notate all interactions to enhance collaboration and improve patient services. To thrive as a Referral Coordinator at Fourth Street Clinic, you'll need a unique blend of skills and experiences. A background in social services or direct patient care, especially working with underserved populations, is essential. You should possess strong oral and written communication skills, allowing you to effectively interact with patients, medical providers, and community partners. Proficiency in using EPIC or other electronic health record systems will streamline your workflow and enhance collaboration. Excellent interpersonal skills are crucial, along with the ability to multitask effectively in a dynamic environment. Being self-motivated and accountable will help you excel in this role, as well as your openness to constructive feedback for continuous improvement.
