Job Description
Care Clinician II Eldercare/BHIP Seeking a forty hour per week clinician to provide assessment, psychotherapy and other clinical services to adults in Washington County. Twenty hours per week will be dedicated to serving clients aged sixty and over utilizing an outreach-based approach primarily in homes. The remainder of work is office-based psychotherapy conducted in the outpatient clinic and a primary care medical office. The clinician will work collaboratively with colleagues to ensure coordinated and collaborative care. EXPEPERIENCE REQUIREMENTS
Specific knowledge of and experience working with the issues of aging is beneficial. A commitment to a strength-based approach aimed at maximizing the client's independence. At ease working independently with knowledge supervision and support are regularly and consistently available. Develop and implement individualized treatment plans for each client ensuring their voice is integrated. Coordinate services and collaborate with colleagues, families and external agencies. Participate in meetings and treatment planning discussions. Complete all required clinical documentation in a timely manner. Established interpersonal skills and the ability to work within a team and effectively communicate with WCMHS colleagues and external entities. EDUCATION and
LICENSURE
Master's degree, licensed or licensed-eligible, with a minimum of one year experience providing psychotherapy required. Must have a valid driver's license, possess an excellent driving record and have access to a safe, reliable and insured vehicle.
