Job Description

Care Coordinator - Full Time Everett, WA www.compasshealth.org Support Youth and Families in Your Community Compass Health is seeking a dedicated Care Coordinator to join our Children's Intensive Services (WISe) program in Everett. You'll be part of a compassionate, community-based team supporting youth with complex behavioral health needs and helping families thrive through individualized, strengths-based care. WISe (Wraparound with Intensive Services) serves Medicaid-eligible youth (up to age 21) by offering behavioral health services in natural environments like homes, schools, and communities. Watch the video What You'll Do As a Care Coordinator on the Mint team, you will: Provide therapeutic support and case management for youth and families. Develop individualized treatment plans using a strengths-based approach. Facilitate resource identification, service monitoring, and crisis intervention. Collaborate with schools, natural supports, and community providers. Participate in the rotation of after-hours emergency response (crisis assessments, outreach, and safety planning). Work across home, office, and community settings, including evenings as needed. What You Bring
BA/BS/BSW
