Job Description

Jewish Family Service is a 134-year-old non-profit organization providing social services to individuals and families in the Jewish and greater communities of the Puget Sound region. For more information about JFS, please visit www.jfsseattle.org We are currently seeking a Temporary Geriatric Care Manager to join our Older Adult Services team. This part-time (0.5 FTE or 18.75 hours per week) position is responsible for providing care management services to older adults and people with disabilities in East King County. The Geriatric Care Manager will provide some or all of the following services: information, assistance, and referral; comprehensive assessment; care management; and community outreach. This is a part-time, six-month temporary position. We welcome and encourage you to apply if you feel connected to JFS' mission. ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide comprehensive assessment of older adults' physical and mental health, home safety, social needs, cultural needs, and financial status, in a linguistically and culturally appropriate manner. Develop care plans in collaboration with clients and help clients achieve the goals outlined in the care plans. Serve as a client advocate and contact clients' healthcare providers, community agencies, etc. to ensure the client's needs are met. Provide information, assistance, and referral to senior clients and their families. Provide ongoing oversight of each assigned case, maintaining regular contact with client (including home visits, telephone checks, annual reassessments, etc.). Coordinate services with other Older Adult Services and JFS staff. Coordinate care for clients when multiple service providers are involved (this may include liaison work with other communal organizations). Maintain accurate and complete documentation in accordance with agency and contractual requirements. This includes recording all client contact in the Salesforce database. Comply with all City of Seattle contractual requirements regarding service provision and documentation. Regular and predictable attendance and punctuality. Attend and participate in monthly OAS clinical consultation and team meetings and other meetings as requested. Other duties as assigned.
QUALIFICATIONS
A master's degree in social work or related field and one year paid social service experience is preferred; a bachelor's degree in social work or related field and at least two years' paid social service experience will be considered. One year of experience in geriatrics is preferred. Licensure as a social worker or mental health counselor is preferred. Must have a valid Washington State Driver's License and access to an insured vehicle. Fluent in English, including excellent written skills in English. Basic math skills, including addition, subtraction, multiplication, and division. Must have knowledge of community resources. Strong interpersonal skills are required. Must be able to work effectively and compassionately with clients who are frail or have physical or cognitive disabilities. Strong interview and assessment skills are needed. Excellent attendance record. Working knowledge of Microsoft Word and Outlook is required Ability to learn to use an electronic database is also required
SALARY:
