Job Description
- Resident Care Coordinator
Job Summary:
- Highgate at Wenatchee, a leading senior living community, is seeking a dedicated and compassionate
- Resident Care Coordinator
- to join our team.
Responsibilities:
- Coordinate and maintain accurate records of resident care, including assessments, care plans, and progress notes
- Collaborate with the care team to ensure that resident needs are met, and concerns are addressed promptly
- Provide emotional support and companionship to residents, as well as assist with daily living activities and personal care
- Monitor and report changes in resident conditions to the care team and family members, as necessary
- Participate in care conferences and contribute to the development of individualized care plans
- Maintain confidentiality and adhere to HIPAA guidelines
