• Responsibilities:

• Coordinate and maintain accurate records of resident care, including assessments, care plans, and progress notes
• Collaborate with the care team to ensure that resident needs are met, and concerns are addressed promptly
• Provide emotional support and companionship to residents, as well as assist with daily living activities and personal care
• Monitor and report changes in resident conditions to the care team and family members, as necessary
• Participate in care conferences and contribute to the development of individualized care plans
• Maintain confidentiality and adhere to HIPAA guidelines
