About Pathways Pathways to a Better Life is a growing Wisconsin-based alcohol and drug treatment and recovery center dedicated to helping individuals and families affected by chemical dependency. Through residential detox, adolescent and adult treatment, day treatment, and sober living programs, we believe in treating the whole person - physically, mentally, emotionally, and spiritually. About the Role We are looking for a compassionate, organized, and people-centered Intake / Discharge Coordinator to ensure every individual who walks through our doors feels welcomed, informed, and supported - and that they leave with the resources and follow-up care they need to continue their journey to recovery.
What You'll Do Intake Responsibilities:
Welcome new residents and guide them through the intake process with care and professionalism Review and complete all required paperwork with incoming residents Scan and distribute documentation to appropriate staff and departments Update electronic records with information gathered prior to and during intake Prepare new resident charts Conduct facility tours for incoming residents Search belongings as needed in accordance with facility policy
Discharge Responsibilities:
Complete all required discharge paperwork with residents prior to departure Return personal items including cell phones and any secured belongings Gather all paper documentation and deliver to the records department Ensure follow-up contact is made with residents after discharge to support continued recovery Who You Are You are warm, welcoming, and able to put people at ease during what can be a vulnerable and emotional time. You are comfortable working with people in crisis, calm under pressure, and able to maintain professional boundaries. You are highly organized, attentive to detail, and take the responsibility of accurate recordkeeping and confidentiality seriously. You believe in the mission of recovery and treat every resident with dignity and respect. Qualifications Experience in a healthcare, social services, or administrative setting is preferred. Strong organizational and communication skills are required. Familiarity with electronic health records or document management systems is a plus.
