- This is a full-time role that will primarily support our Louisville-Downtown clinic.

Identify and assess patient's clinical issues and health care needs Record physical findings, and formulates plan and prognosis, based on patient's condition and the doctor's plan of treatment Evaluations and Re-evaluations of established patients Refers patients to Physician for consultation or to specialized health resources for treatment when needed. Discusses the case with Physician and other health professionals to prepare a comprehensive patient care plan Performs pain management injections for specific diagnoses according to practice protocols Prescribe/Order as medically indicated- Diagnostics
