Responsibilities Review and analyze Medicare patient charts to verify completeness, accuracy, and compliance with regulatory standards. Utilize Electronic Health Record (EHR) systems to access, evaluate, and document findings efficiently. Identify discrepancies or errors in medical documentation and communicate findings to relevant teams for correction. Maintain detailed logs of chart reviews, including notes on compliance status and areas requiring follow-up. Collaborate with clinical staff to clarify documentation issues and improve record quality processes. Stay updated on Medicare regulations, coding guidelines, and best practices related to medical record review. Skills Prior experience working in a medical office environment or healthcare setting is highly valued. Proficiency with Electronic Health Record (EHR) systems for chart access and documentation review. Strong understanding of medical terminology to accurately interpret clinical notes and records. Experience reviewing medical records for completeness, accuracy, and regulatory compliance. Excellent attention to detail with the ability to identify inconsistencies or errors in complex documentation. Effective communication skills for collaborating with clinical teams and documenting review findings clearly. Knowledge of Medicare guidelines, billing practices, and coding standards is a plus.
