Job Summary Join our dynamic healthcare team at Springdale Post Acute as a Case Manager Assistant, where your expertise will drive exceptional patient outcomes through coordinated care and comprehensive management. In this vital role, you will assess patient needs, develop individualized care plans, and collaborate with multidisciplinary teams to ensure seamless transitions across care settings. Your will be required to navigate complex medical situations, optimize resource utilization, and promote efficient discharge planning. We are seeking a motivated professional committed to delivering compassionate, patient-centered service while adhering to healthcare regulations and standards. Responsibilities Conduct thorough assessments of patients' medical, psychological, and social needs utilizing EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems Develop and implement personalized care plans aligned with clinical guidelines Coordinate with facility's Interdisciplinary Team (IDT)—to ensure effective discharge planning and utilization review Monitor patient progress through ongoing communication with healthcare providers, social workers, behavioral health specialists, and family members Manage intake processes for new patients requiring complex case management services in post acute care settings Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information at all times Assist in utilization management by reviewing medical necessity criteria and supporting documentation for appropriate resource allocation Experience Familiarity in discharge planning, utilization review, and managed care processes Knowledge of medical terminology and utilization management standards Must possess the ability to make independent decisions, follow instructions Skilled in navigating EMR/EHR systems while maintaining HIPAA compliance Ability to generate written communication that is clear, concise, and well-organized Excellent oral communication skills to collaborate effectively across multidisciplinary teams in fast-paced hospital settings Experience working in Skilled Nursing (SNF) settings preferred Join us to leverage your clinical expertise in a role that challenges you to deliver impactful patient care while advancing your professional growth. Pay:
$19.00 - $23.00 per hour
