DOE Job Description The Case Manager is responsible for coordinating and supporting patient care services throughout the continuum of treatment. The Case Manager works collaboratively with the clinical and medical teams to ensure patients receive appropriate services, resources, and support in alignment with individualized treatment plans and program requirements. All duties are performed in accordance with organizational policies, ethical standards, and applicable federal, state, and regulatory requirements. Education and Training Requirements High school diploma or GED required; Bachelor's Degree in Social Services, Psychology, Human Services, or a related field, or equivalent experience in behavioral health or substance use disorder treatment encouraged but not required. Current CADC, CATC, SUDCC, or other recognized California Substance Use Disorder Counselor credential required. Knowledge of substance use disorder treatment, levels of care, and case management best practices. Strong organizational, communication, and documentation skills. Current TB clearance. Current First Aid/CPR certification. Ability to maintain patient confidentiality and handle sensitive information appropriately. Valid California driver's license preferred. •RADT will NOT qualify. Essential Duties and Responsibilities These are the most significant performance factors involved with this position and will be the principal factors on which the employee will be evaluated: Provide weekly case management services to assigned patients in accordance with individualized treatment plans. Complete and document weekly case management notes accurately and within required timeframes. Coordinate services related to housing, legal matters, employment, education, medical care, benefits, and aftercare planning. Collaborate with clinical, medical, and support staff to ensure continuity of care throughout treatment. Participate in multidisciplinary treatment team meetings and complete case conference progress notes for assigned patients. Maintain accurate, timely, and audit-ready documentation in compliance with company policies and regulatory standards. Advocate for patient needs while maintaining appropriate professional boundaries and ethical standards. Train and support case managers in completing case conference progress notes and maintaining documentation standards. Educate patients on available community resources and assist with discharge planning and referrals. Maintain compliance with HIPAA, DHCS, Joint Commission, payer requirements, and company policies and procedures. Attend and participate in staff meetings, in-service trainings, and continuing education as directed by the Program Director or Clinical Director. Perform other duties as assigned.
