Program Case Manager is responsible for providing comprehensive, client-centered case management services to individuals enrolled in the Modesto Gospel Mission's Recuperative Care Program. This role supports individuals experiencing homelessness who are recovering from acute medical conditions by coordinating medical care, behavioral health services, housing navigation, and community-based support. The Case Manager ensures compliance with Department of Health Care Services (DHCS) and Managed Care Plan (MCP) requirements under Cal AIM, while promoting positive health outcomes, housing stability, and long-term self-sufficiency. Responsibilities
  • Conduct comprehensive intake assessments, including medical, behavioral health, housing, and social determinants of health.
  • Develop and implement individualized, person-centered care plans in collaboration with clients and interdisciplinary team members.
  • Coordinate medical appointments, specialty care, and follow-up services to ensure continuity of care.
  • Provide ongoing case management support, including goal setting, progress monitoring, and care plan updates.
  • Facilitate discharge planning to stable housing or appropriate placement with necessary supports in place.
  • Monitor client progress during recuperative stay and communicate updates with healthcare providers and MCPs.
  • Assist clients with medication adherence, appointment compliance, and understanding discharge instructions.
  • Support activities of daily living (ADLs) as appropriate within scope of non-clinical staff.
  • Identify and respond to changes in client condition, escalating concerns to clinical staff as needed.
  • Maintain accurate, timely, and compliant documentation in electronic record systems.
  • Ensure all services are documented in accordance with DHCS, Cal AIM, and MCP billing and documentation standards.
  • Track and report client outcomes, service utilization, and program metrics as required.
  • Participate in internal audits and quality assurance activities to ensure program compliance. Experience
  • Proven experience in social work, human services, behavioral health, or related field (Preferred)
  • Ability to work effectively with individuals recovering from injury, illness, and or disabilities
  • Excellent organizational and communication skills
  • Demonstrated proficiency in relationship management and team collaboration
  • Knowledge of best practices in supporting individuals recovering from injury, illness, and or disabilities
  • Case management experience (Preferred)
