Job Description

We are seeking a dedicated and compassionate Care Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their treatment journey. This position requires strong clinical knowledge and excellent communication skills to effectively collaborate with patients, families, and healthcare professionals. Responsibilities Coordinate with multidisciplinary teams to ensure comprehensive patient care across various settings, including inpatient, outpatient, and emergency medicine. Conduct assessments of patients' medical records to identify necessary services and resources. Facilitate discharge planning for patients transitioning from acute care or critical care settings. Provide education and support to patients and their families regarding treatment options, resources, and follow-up care. Monitor patient progress and adjust care plans as needed based on ongoing assessments. Advocate for patients' needs within the healthcare system, ensuring they receive appropriate infectious disease care and other necessary services. Maintain accurate documentation of patient interactions, care plans, and outcomes in compliance with healthcare regulations. Requirements Patient care experience such as LPN or Medical Assistant is preferred. Experience in inpatient settings, geriatrics, or acute care environments is preferred. Knowledge of medical records and patient service protocols. Excellent interpersonal skills with the ability to communicate effectively with diverse populations. Ability to work collaboratively within a multidisciplinary team to achieve optimal patient outcomes. Strong organizational skills and attention to detail in managing multiple cases simultaneously.

$23.00 - $26.00 per hour Expected hours: 40.00 per week
