Job Description

Duties Prioritize patient care needs upon initial visit and addresses emerging issues Identifies and assists with the follow- up of high- risk patients in acute care settings, skilled nursing facilities, custodial and ambulatory settings. Consults with providers and other team members to ensure that care plan is successfully implemented Coordinates treatment plans with the care team and triages interventions appropriate to the skill set of each team member Uses protocols and pathways in line with the established disease management and care management programs and approved by medical management to optimize clinical outcomes Monitors and coaches patients using techniques of motivational interviewing and behavioral changes to maximize self-management. Oversees provisions for discharge from facilities including hospital follow-up appointments, home health and social services, transportation, etc. to maintain continuity of care. Discusses DPoA and Advanced Directives status with PCP when applicable Provides accurate information to patients and families regarding HMO benefits, community resources, referrals, and other related issues Adheres to departmental policies and procedures as approved by CFM. Assist in the assessment of clinical operations and makes recommendations as necessary Quality measures Always demonstrate service excellence Other duties as assigned Skills Strong communication and interpersonal skills Ability to work independently and as a part of a multidisciplinary team Excellent organizational and problem-solving skills Knowledge of health care systems, insurance and community resources Proficiency in using EHR and other software symptoms Extensive clinical experience Excellent phone etiquette Excellent attention to detail A demonstrated commitment to high professional ethical standards and a diverse workplace Ability to adapt to a fast paced continually changing business and work environment while managing multiple priorities Compassionate and able to handle stress Requirments Licensed Clinician (LPN or RN) required. Current state licensure and clinical experience preferred. Our Case Managers play a crucial role in providing comprehensive care coordination for patients. They are responsible for assessing patients' needs, developing individualized care plans, coordinating communication between healthcare providers, monitoring patient progress, and advocating for patients' rights. The ideal candidate will have acute care experience, strong medical records management skills, and the ability to work collaboratively with a multidisciplinary team. A solid understanding of medical terminology, and patient assessment is also required.
