A Clinical Case Manager coordinates medical care and provides advocacy to help people get the best and most affordable care. Helps patients navigate insurance and hospital bureaucracies and access information. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment and travel to different doctors' offices, hospitals or a patient's home.

Schedule:
7a-7p or 7p to 7a Would prefer Certified Peer Specialist but qualifications count. 36 hours a week speciy night or day shift also hiring for part time and weekends.

Job Description:
Provides Skills training and connects individuals with resources that will allow Individuals to live, learn, work, and socialize in their environments of choice. Provide service interventions focusing on wellness, identification of strengths, self-advocacy in connection with goals as identified by the individual's assessment and treatment plan. Provides services to designated/assigned peer groups. Attends scheduled treatment team meetings and staff meetings. Completes and submits all clinical documentation/weekly progress notes. Ensures assigned caseload of individuals has a active authorization before providing services. Maintains certification as a CPS (Certified Peer Specialist). Collaborates with psychiatrist, nurse practitioners, Program Directors, within SBHS. Must be dependable/flexible.
Minimum Qualifications:
Must have a High School Diploma or GED. Must have a valid driver's license and a good driving record.
Preferred Qualifications:
Certified Peer Specialist. One year of experience as a case manager. Experience in documentation through electronic medical records.
