The WCMS shall be accountable for 1) Understanding and complying with the Ryan White Part C Grant expectations and requirements, 2) Providing case management services as part of a multidisciplinary team to eligible patients accessing services at the Wellness Center and 3) Maintain an understanding of HIV treatment and care guidelines 4) Supervise Medical Case Management team. The WCM works closely with Wellness Center Staff, as well as team members from Emerald clinical support, reception, pharmacy, and providers. Schedule - 8:00am-5:00pm, Monday-Friday. Compassionate care is at our core! Duties and Responsibilities Identify, access, and enroll individuals who would benefit from available services. Conduct face to face HIV medical case management assessments to identify factors that impact patient ability to adhere to their medical plan. Oversees Medical Case Management team compliance with Ryan White, HRSA, IDHW, and organizational requirements. Assists with preparation for Ryan White program monitoring activities, audits, and
HRSA/IDHW
site visits. Develop individual service plans based on assessments and coordinate the development of the services plan with the client's participation. Update the ISP at a minimum of every six months. Coordinate service delivery to ensure continuity of care. Maintain frequent contact with providers and clients to ensure that services are provided as indicated in the service plan. Provide documentation and data entry as required to maintain patient records. Assist patients with accessing resources (oral health care, medication, emergency financial assistance, housing, etc.) Maintains current and thorough knowledge of HIV, including progression, transmission, medications, monitoring and Ryan White Grant. Participates in quarterly
HIV COQM
meetings and PDSA/QM projects. Provide documentation, complete annual and bi-annual IDHW recertification's as necessary and complete data entry into CAREWare and the EMR as required to maintain patient records. Reviews team CAREWare data and documentation for accuracy, completeness, and compliance; collaborates with staff to address identified deficiencies. Counsels eligible patients regarding available benefits and entitlements; facilitates referrals to Full Circle Health, private and public resources or programs such as Medicaid, Medicare, State Pharmacy Assistance Programs, Pharmaceutical Manufacturers Patient Assistance Programs and other State or local health care and support services to ensure . Ensure Ryan White is the payer of last resort. Attends scheduled team and interdepartmental meetings as directed. Perform other duties as requested. Education:
Degree in Social Sciences, Nursing, Social Work, Psychology or related field from an accredited four-year college or university, preferred.
Experience:
3 or more years working in a social service organization or healthcare setting providing service delivery. 2 or more years working in a supervisory role. Credentialing Meets established Credentials Committee requirements for Medical Case Management scope of practice including fully credentialed, and privileged as well as the ability to pass a background check.
Other Knowledge, Skills and Abilities:
Excellent verbal and written communication skills Bilingual, preferred.
