Medical Case Manager

Responsibilities Identify clients who may benefit from intensive case management services for health-related needs or other vulnerabilities, as appropriate. Screen for eligibility and enroll in program. Develop individualized service plans in collaboration with clients to work towards self-sufficiency Track client progress on goals and complete regular assessments Support clients in navigating healthcare and benefits systems to obtain coverage, benefits and supportive services Coordinate medical care including appointment scheduling, facilitating communication between providers, addressing medication and medical supply needs, and ensuring follow-up care Provide education on transportation resources, such as public transit training, scheduling rides through insurance and applying for ADA paratransit services Accompany clients to appointments as needed Help clients build confidence and skills to move towards independence Strengthen existing service networks and community linkages and identify new resources/partners Maintain accurate case files, notes and data using RefugeeOne's electronic and file systems Complete required reporting accurately in a timely manner These responsibilities are not exhaustive and are subject to regular review. Qualifications Must have a valid driver's license and use of a reliable car Bilingual fluency in any language is helpful (Arabic, Rohingya, Spanish, etc.) Bachelor's degree preferred Experience working with individuals from diverse cultural communities preferred Exceptional problem-solving skills, including the ability to develop and follow through with practical solutions Able to maintain a calm, professional demeanor and take action in stressful situations Demonstrates excellent organizational and time-management skills and able to juggle multiple priorities and responsibilities Experience working with individuals from diverse cultural communities preferred Must be able to have schedule flexibility Experience using Salesforce or other CRM preferred Must have strong computer skills including working knowledge of Microsoft 365 (SharePoint, Outlook, Word, Excel and Teams)

Compensation :

The base annual salary range for this role is $47,000-$48,500 commensurate with education and experience. A generous benefits package is offered with full-time employment.
