Job Summary :

The Adolescent Support Clinician serves as a bridge role fulfilling both therapeutic and case management duties. This role can expect to complete client biopsychosocial assessments and provide case management for students and their families. Case management duties may include coordinating ancillary care for legal appointments, vocational issues, insurance coordination, external referrals, and general life skills. Further, this role will remain regularly engaged with the medical and clinical teams to support continuity of care needs during and after treatment. Additionally, this role will assist in times of high acuity or census with facilitating clinical content and/or process groups. The Adolescent Support Clinician will document all of the client activities that s/he is responsible for in the electronic medical record.

Duties and Responsibilities:

The Adolescent Support Clinician is involved in client treatment processes in the following ways: Completes Bio-Psycho-Social Assessment (BPS) and all related documentation in accordance with policies, procedures, and protocols. Maintains a case management caseload and provides one-on-one sessions weekly with clients and their families to support their case management needs. Collaborates with interdisciplinary staff to support continuity of care clinically and medically. Responsible for conducting therapy groups weekly. Completes individual case management assessment Maintains contact with important collaterals (i.e. family, probation, school) within the scope of their case management work and release of information. Assures proper client consents and other treatment documents are completed. Maintains contact with Clinical Director and Program Director regarding caseloads. Responsible for attending and participating in mandatory weekly staff meetings to discuss client care and all aspects of administration. Knowledge of policy and procedure manual relating to program protocol. Other duties as assigned. Adolescent Support Clinician performs and maintains professional abilities in the following areas: Maintain and adhere to all Federal, State, and County regulations regarding client confidentiality and staff code of ethics. Perform tasks the supervisor assigns including, but not limited to, schedule, case load, and deadlines. Continuous training in critical areas, maintaining CEU expectations Crisis intervention—recognizing emotional stress and utilizing resources to resolve problems. Client education—provide current alcohol/drug information and available resources to clients, groups, and families.

Qualifications:

Certified Alcohol and Drug Counselor (CADC), Licensed Social Worker (LSW), or Licensed Professional Counselor (LPC). Minimum of one (1) year previous experience in the addiction field. Ability to prioritize and provide appropriate treatment protocols. Maintains current certification for this position:
