Job Description
Company Overview St. John Center for Homeless Men is dedicated to empowering homeless men by addressing the barriers they face in achieving self-sufficiency and stable housing. Our mission is to provide essential services, including shelter, social support, and collaborative partnerships, helping individuals find hope and move forward in their lives. Description The Clinical Social Worker will support St. John Center's Outreach Team by providing behavioral health and clinical case management services to individuals and families experiencing homelessness. This position works collaboratively with Outreach Specialists and Housing Navigators to support unsheltered outreach efforts, diversion services, housing stabilization, and connections to community-based clinical supports. The Clinical Social Worker will complete client intakes, biopsychosocial assessments, disability verifications, referrals to behavioral and mental health providers, Releases of Information (ROIs), and other clinical documentation necessary to support housing and treatment goals. This role will also support the St. John Center Diversion Program by assisting clients in identifying safe alternatives to shelter entry and strengthening natural support systems. St. John Center, a second chance employer, operates from a harm reduction and housing-first philosophy rooted in dignity, compassion, and person-centered care. The Clinical Social Worker reports directly to the Outreach Team Manager and receives clinical supervision from the Clinical Director.
