  • Responsibilities:
  • Coordinate patient care from admission through discharge
  • Collaborate with physicians, nurses, therapists, and interdisciplinary teams
  • Develop and manage individualized discharge plans
  • Conduct patient and family education
  • Facilitate care conferences and treatment plan discussions
  • Monitor patient progress and identify barriers to successful discharge
  • Coordinate community resources and post-discharge services
  • Qualifications:
  • Active RN, Social Work, Occupational Therapy, Physical Therapy, Speech Therapy, or Rehabilitation Counseling license/certification (where applicable)

  • $45.
