Job Description

Job Summary We are seeking a dedicated and qualified professional to join our team. The ideal candidate will possess a strong background in case management and counseling, particularly within the realms of addiction and mental health. This role involves providing comprehensive support to patients, ensuring their needs are met through effective service delivery and adherence to best practices in patient care. Duties Conduct thorough assessments of patients to determine their needs and develop individualized care plans. Provide case management services, coordinating with healthcare providers to facilitate patient access to necessary resources. Offer addiction counseling and mental health support to patients, utilizing evidence-based practices. Maintain accurate medical records in compliance with HIPAA regulations, ensuring confidentiality and security of patient information. Assist in discharge planning for patients, ensuring a smooth transition from clinical settings to home or community-based care. Collaborate with clinic staff and managed care organizations to optimize patient outcomes and service delivery. Participate in emergency medicine situations as needed, providing immediate support and intervention. Qualifications Bachelor's degree in social work, psychology, or a related field; relevant certifications are a plus. Proven experience in case management is highly desirable. Strong understanding of mental health counseling principles and practices. Familiarity with medical records management and HIPAA compliance is essential. Excellent communication skills, both verbal and written, with the ability to interact effectively with diverse populations. Ability to work collaboratively within a team-oriented environment while also demonstrating strong independent decision-making capabilities. Commitment to providing high-quality patient service and support. Strong knowledge of 1915i services and person-centered planning. Familiar with local MCO systems, Trillium health resources preferred. Valid license and reliable transportation.
