Job Description

NFI North is seeking a seeking a full-time Clinical Case Manager for our Pathways Transitional Housing Program in Concord, NH. This program is a community residence serving 16 individuals and includes a day treatment component. We provide intensive clinical, medical, vocational and residential services to adult men and women with mental health challenges working towards independent living. Our goal is to help individuals successfully transition into the community while maintaining their independence in the least restrictive environment possible.
Responsibilities:
Provide case management and clinical services to assigned participants, administrative duties and clinical support for participants, including supported employment as well as providing clinical training and support for staff. Responsible for all financial information and benefits management.
Qualifications:
Bachelor's Degree in Human Service field with four years related experience and/or training. Experience with supported employment activities and the participant population preferred.
