This energetic position involves coordinating services for patients , including hospitals, emergency departments, and outpatient clinics. Your expertise will help streamline patient assessments, discharge planning, and utilization management to ensure seamless transitions of care. We are seeking a motivated professional passionate about delivering exceptional patient service and improving health outcomes through proactive case management. Responsibilities Conduct intake and comprehensive patient assessments to determine needs of patient. Collaborate with multidisciplinary teams including social workers, physicians, nurses, and mental healthworkers to develop personalized care plans. Manage discharge planning processes by coordinating services. Oversee utilization management activities to ensure appropriate level of care while adhering to healthcare policies like HIPAA and medical records regulations. Maintain accurate medical records and documentation of case progress, assessments, and interventions. Provide patient-centered support through behavioral health counseling, or mental health services as needed.
Qualifications Qualifications:
Bachelor's degree in Human Services field, or -Associate's degree and 2-year experience in providing Human Services, OR -5 years of Human Services experience Knowledge of medical terminology, physiology, and diagnostic evaluation techniques essential for effective case management. Familiarity with managed care processes, inpatient and outpatient clinic operations, and post-acute care coordination. Experience with patient assessment tools, discharge planning procedures, and utilization management protocols. Excellent communication skills to liaise effectively with healthcare providers and patients while maintaining confidentiality per HIPAA guidelines. Ability to handle complex cases involving mental health counseling, addiction support, or pediatric populations with sensitivity and professionalism. $17.00 - $19.00 per hour
