Case Manager Confidential Saddle Brook, NJ Job Details Full-time $40,000 - $45,000 a year 5 hours ago Benefits Health insurance Dental insurance Paid time off Vision insurance Qualifications Master's degree in social work Teamwork Microsoft Excel Master's degree Typing Care plans Time management Full Job Description Job Overview We are seeking a qualified Master's-level professional to join our multidisciplinary clinical team as a Case Manager supporting clients enrolled in our Substance Use and Mental Health treatment programs, including Partial Care (PC), Intensive Outpatient (IOP), and Outpatient (OP) levels of care. The Case Manager is responsible for individual service coordination for clients enrolled in treatment, with primary responsibility for clients of the highest acuity. This includes supporting clients in a successful transition back into the community or to another care facility or program. In collaboration with clinical staff, the Case Manager supports the successful integration of clinical treatment, medical care, housing, income and employment assistance, and other services within individual treatment plans. Essential Responsibilities Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. Handle case assignments, draft case management service plans, review case progress, and determine closure of case management cases. Enter data into and manage an outcomes tracking database. Support clients in achieving wellness and autonomy. Facilitate multiple aspects of care, including case coordination and information sharing. Perform BioPsychoSocial assessments on all admitting clients. Develop effective working relationships and cooperate with psychiatric and behavioral health teams throughout the entire case management process. Arrange abeyances with social services, health, and governmental agencies. Maintain consistent engagement with clients to monitor progress and ensure satisfaction. Record case information, complete all necessary forms accurately, and produce statistical reports. Assess and address motivational and psychosocial issues. Facilitate group therapy. Adhere to professional standards as outlined by applicable protocols, rules, and regulations. Perform other duties as assigned by supervisor(s). Qualifications Master's degree in Social Work (required). Comprehensive knowledge of case management principles, healthcare management, and reimbursement. Effective oral and written communication skills. Strong organizational and time management skills. Familiarity with Windows-based computer software, including proficiency in Microsoft Word, Excel, and PowerPoint. Experience with Kipu is preferred. Proficiency in typing on a computer keyboard. Strong problem-solving skills and the ability to manage multiple priorities. A compassionate disposition and demonstrated ability to work collaboratively within a team.
