At our core, we aim to increase opportunities for health and wellness and to ward off health and social problems that can lead to a lifetime of illness and dependency. Children•s Futures is a 501 (c) 3 nonprofit organization whose mission is to improve health and development outcomes for children and their families. To achieve our mission, we formally collaborate with schools, social service providers, health care agencies, faith institutions, civic associations, employers and others around school-readiness and success strategies that have the greatest positive impact on children's immediate health needs, as well as long-term development. We are looking for a Nurse Home Visitor to join our dedicated team to serve first time mothers enrolled in Children•s Futures Nurse-Family Partnership program.
Responsibilities:
Provide comprehensive services to women and their families. Visit clients in their home. Oversee documentation, data entry, and reporting to ensure compliance with Healthy Families program and funder requirements. Develop and operationalize program structure, workflows, policies, and procedures aligned with model requirement Assess physical, emotional, social and environmental needs of the women and their families. Supervise Case Managers and support staff, including coaching, performance management, and professional development. Strengthen partnerships with community-based organizations, healthcare providers, and referral agencies. Maintain confidentiality and document accurately. Develop therapeutic relationships with the women and their families. Provide education, support and referral resources to assist the women and their families with attaining their targeted goals. Participate in management meetings and contribute to organizational strategy.
