This position works directly with individuals receiving substance use disorder and mental health treatment services and plays a critical role in helping clients remain engaged in treatment, access resources, and achieve long-term recovery goals. The successful candidate will work closely with counselors, medical providers, referral sources, probation departments, and community organizations to help clients address barriers that may interfere with treatment success. This position is ideal for someone who enjoys building relationships, advocating for others, and making a meaningful impact in the lives of individuals and families affected by addiction. Essential Responsibilities Provide case management services to clients participating in outpatient treatment programs. Assist clients with accessing housing, employment, transportation, healthcare, educational opportunities, financial assistance, and community resources. Coordinate care with treatment providers, referral sources, probation officers, and other community partners. Monitor client progress and support achievement of treatment plan goals. Maintain regular client contact and provide ongoing support throughout treatment. Participate in treatment team meetings and collaborate with clinical staff. Complete required documentation accurately and within established timelines. Maintain confidentiality and compliance with
HIPAA, 42 CFR
Part 2, Medicaid requirements, and agency policies. Advocate for clients while promoting accountability and personal responsibility. Qualifications Required Active Ohio CDCA certification. High school diploma or equivalent. Valid driver's license and reliable transportation. Strong communication and organizational skills. Preferred Experience working in substance use disorder treatment, mental health services, healthcare, corrections, social services, or related fields. Knowledge of community resources throughout Northwest Ohio.
