Our brand new dual-diagnosis behavioral health treatment center is seeking a compassionate, organized, and detail-oriented Case Manager to support clients navigating both primary mental health and substance use disorders. The Case Manager plays a crucial role in coordinating care, advocating for client needs, and ensuring each individual receives comprehensive and continuous support throughout treatment. Key Responsibilities Conduct biopsychosocial assessments and assist in developing individualized treatment plans. Serve as the primary point of contact for clients regarding care coordination and progress updates. Facilitate communication between clinical, medical, admissions, and psychiatric teams to ensure cohesive care delivery. Maintain accurate and timely documentation in accordance with facility, state, and accreditation standards (e.g., Joint Commission/CARF). Coordinate discharge planning, including aftercare referrals, community resources, outpatient appointments, and housing or transportation needs. Provide ongoing case management sessions focused on goal-setting, progress review, and barrier reduction. Communicate with family members or referral sources as appropriate and authorized. Participate in interdisciplinary team meetings and contribute to treatment planning discussions. Support crisis intervention efforts and collaborate with clinical leadership when clients require elevated support. Ensure compliance with confidentiality standards (HIPAA) and ethical case management practices. Qualifications Bachelor's degree in Social Work, Psychology, Behavioral Health, or a related field is preferred. Prior experience in behavioral health, substance use treatment, or dual-diagnosis programs strongly preferred. Strong understanding of levels of care (Detox, Residential) and the needs of dual-diagnosis clients. Excellent communication, documentation, and time-management skills. Ability to work in a fast-paced environment and collaborate with multidisciplinary teams. Familiarity with electronic medical records (EMR) systems; experience with KIPU is a plus.
