Apply Clinical Case Manager - Abe's Place WM Aliquippa, PA Apply Job Type Full-time Description Gateway Rehab Center (GRC) is seeking a Clinical Case Manager to support the day-to-day operations of the Withdrawal Management program. This role is responsible for conducting assessments, assisting patients with appointment scheduling, and collaborating with the treatment team to develop and implement continuing care plans. The position also coordinates continuing care services, provides exceptional support to patients, and helps ensure a smooth transition to the next appropriate level of care throughout the recovery journey. Responsibilities Serves as a member of the program's interdisciplinary treatment team. Assist patients with scheduling medical, legal, and other necessary appointments. Coordinates continuing care appointments and services prior to discharge. Completes timely and accurate documentation in patient charts related to intake assessments, case management, treatment activities, and continuing care coordination. Facilitate group sessions as assigned. Completes biopsychosocial assessments and all required intake paperwork. Provides comprehensive written and verbal follow-up to referral sources during treatment and at discharge. Maintains professional communication and working relationships with referral sources, outside treatment providers, and community agencies. Coordinates and communicates with other Gateway programs and departments to support continuity of care. Provides oversight of patient daily activities as needed, including unit supervision and recreational activities. Provides direct patient care as needed, including crisis intervention. Knowledge, Skills, and Abilities Understanding of 12 step recovery programs and the recovery process preferred. Computer literacy preferred. Excellent people skills and organizational skills. Requirements Bachelor's Degree Additional Requirements Valid driver's license with favorable driving record. Obtain CPR and First Aid certification. Pass PA Criminal Background Check. Obtain Childline and FBI clearances. Pass Drug Screen 2-Step TB Test This position requires mandatory staffing coverage when necessary to provide a safe treatment environment for Gateway residents.
