Full Job Description Organizational Overview:
Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to protecting the health and welfare of the homeless, drug users, sex workers, and the uninsured. PPP works to provide safe and humane alternatives to the war on drugs, working to reduce the prevalence of HIV and Hepatitis C by linking people to the health resources they need. PPP meets people where they are, providing information and access to resources with dignity and respect. Using a harm reduction approach, PPP offers low-threshold, culturally sensitive, non-judgmental services to address the health and social service needs of people in Philadelphia. PPP promotes harm reduction through mobile medical care, sterile syringe exchange, referrals to social services, and comprehensive prevention case management services.
Program Overview:
Philly Home is a low-barrier shelter with multiple agencies coordinating services on site. The PPP Philly Home Medical Team is a multi-disciplinary team of Nurse Practitioners, Registered Nurses, and Case Managers. The team provides Stabilization Primary Care, and medication for opioid use disorder (MOUD), and medical care coordination to participants residing on the shelter floor. The unit operates from a harm reduction lens, asking how the program can better fit individual recovery needs rather than how individuals can better fit the program.
Position Duties and Responsibilities:
- Work collaboratively with all members of the Prevention Point Philadelphia Team.
- Assist in providing case management services at the front desk in fast paced MOUD clinic.
- Manage target engagement list and engage participants about connecting for the recommended services
- Triage Participant needs at the Front Desk - both Medical and Case Management needs
- Manage the waiting room/triage flow via Teams Chat - manage queue for services
- Use Athena schedules to check patients into Provider/Nursing/CM schedules
- Support and build rapport with patients while waiting for services, and engage about services offered on the unit.
- Sign out resources distributed to participants
- Engage participants in meaningful and frank discussions about risk behaviors, including overdose and return to use risk. Help participants identify triggers and supports to use to address risk of returning to use.
- Refer and link participants to appropriate mental health and medical services and encourage adherence to treatment/appointments.
Conduct initial engagement with all participants coming to engage in services on floor Support managing outside/locker area utilized by participants Work with clients to identify biopsychosocial goals, such as housing, legal assistance, mental health support, substance use treatment and medical treatment. Collect vitals and screen urine samples for individuals interested in receiving care.
- Facilitate referrals to social work services for individuals not connected to the other Navigation services at PPP & ensure documentation of linkages to services.
- Attends and completes all required trainings.
- Keep accurate and comprehensive documentation in electronic health record. Submit documentation in a timely fashion.
- Responsible for delivering comprehensive integrated services, encompassing testing, PrEP navigation, and HIV/HCV case management.
- Remain flexible and undertake additional tasks as directed by the supervisor.
- Support team with admin tasks and monitor inventory
Minimum Qualifications and Preferred Skills:
- Knowledge of clinical documentation preferred
- Ability to use harm reduction approach with staff and participants
- Must be able to document data in electronic health record database
- Ability to work independently and as part of a team
- Fluency in English and Spanish (preferred)
- Bachelor's Degree in human service related field or equivalent of at least 1 year work experience in case management or care coordination
- Knowledge and experience of social service resources with an understanding of behavioral health issues
- Knowledge of and experience working with persons experiencing homelessness, substance use, and mental health issues
- Commitment to a harm reduction philosophy including meeting people where they are and creating a safe, non-judgmental environment
Other Requirements:
- This position includes the normal physical demands of a community-based health program, including lifting/moving up to 25lbs, bending, and carrying. Other physical requirements may include walking and standing for long periods and sitting for up to 30 minutes
