is seeking a compassionate and organized Case Manager to join our team.   • Position Overview The Case Manager is responsible for coordinating and overseeing home-based health services for members.
This role involves assessing needs, developing individualized service plans, monitoring services, and collaborating with healthcare providers, members, and families to ensure high-quality, continuous care.
Qualifications Education:
Bachelor's degree or Minimum of one year paid professional experience working with aging or disabled populations performing case management functions such as assessment, planning, implementation, monitoring, and reassessment. Case management Providers must meet the following minimum qualifications for
CMs:
Baccalaureate degree and one year paid professional or technical experience
  • providing direct care services to aging or disabled populations obtained before or after receipt of degree; o Registered Nurse (RN) with an active Oklahoma license as verified on the Oklahoma Board of Nursing website
  • , with one year paid professional or technical experience
  • providing direct care services to aging or disabled populations obtained before or after receipt of degree; or Licensed Practical Nurse (LPN) with an active Oklahoma license as verified on the experience
  • providing direct care services to aging or disabled populations obtained before or after receipt of degree. Physical Therapist Assistant (PTA) with an active Oklahoma license as verified on the Oklahoma Medical Board website, with one year paid professional or technical experience
  • providing direct care services to aging or disabled populations obtained before or after receipt of degree. Certified Occupational Therapy Assistant (COTA) with an active Oklahoma license as verified on the Oklahoma Medical Board website, with one year paid professional or technical experience
  • providing direct care services to aging or disabled populations obtained before or after receipt of degree. Paid technical experience may include, but is not limited to: Certified Nursing Assistant, Certified Medical Assistant, Certified Home Health Aide, or Personal Care Assistant (PCA) experience.
  • For nurses with a multi-state license issued by a state other than Oklahoma, the license shall be verified on the Nursys National Licensure and Disciplinary Database website.
Advantage Case Management Certification, preferred
Travel Requirement:
Travel within Cleveland, Seminole, Pottawatomie, Pontotoc, and Lincoln counties
Key Responsibilities:
Assess member needs, abilities, and goals Develop and implement individualized ADvantage service plans Coordinate services and ensure continuity of care Monitor service delivery and member progress Identify high-risk situations and intervene appropriately Educate and orient members and families regarding available services Adjust care plans to meet evolving member needs Maintain accurate and compliant documentation Ensure confidentiality according to applicable laws and regulations Monitor plan costs and justify expenditures Assist with in-service education for field staff Perform additional duties as assigned by management
Knowledge, Skills & Abilities:
