We're looking for a compassionate, relationship-driven Clinical Liaison to help connect patients with the specialized inpatient rehabilitation services that can transform their recovery. If you're a clinician who enjoys building relationships, collaborating with healthcare teams, and making a meaningful difference in patient outcomes, we'd love to meet you. What You'll Do As our Clinical Liaison, you will: Build and maintain strong relationships with physicians, case managers, discharge planners, and referring hospitals. Assess patients for admission to our inpatient rehabilitation program using established clinical criteria. Educate patients, families, and referral partners about the benefits of inpatient rehabilitation. Coordinate smooth patient transitions from acute care to rehabilitation. Collaborate with physicians and interdisciplinary teams to ensure appropriate admissions. Represent Acadiana Rehabilitation Hospital in the healthcare community through outreach and networking.
Qualifications Required:
Current licensure as an RN, LPN, PT, OT, SLP, or other qualified healthcare professional. Minimum of two years of clinical experience in acute care, rehabilitation, case management, or a related healthcare setting. Excellent communication, relationship-building, and organizational skills. Ability to work independently while managing multiple priorities.
Preferred:
Previous Clinical Liaison or rehabilitation admissions experience. Knowledge of inpatient rehabilitation admission criteria and payer guidelines. Experience working with physicians, hospital case managers, and referral sources.
