Position Title:
Licensed Counselor - Assertive Community Treatment (ACT)
Location:
Colorado Springs, CO (In-person, community-based)
FLSA Status:
EXEMPT. This position is classified as Exempt under the Fair Labor Standards Act and is not eligible for overtime pay.

Diversus Health is seeking a Licensed Counselor to join our Assertive Community Treatment (ACT) Team, providing intensive, community-based mental health support for adults with Serious and Persistent Mental Illness (SPMI). This is not your typical office job you'll work directly in the community, supporting clients where they live, work, and thrive. You'll be part of a dynamic, multidisciplinary team, delivering therapy, crisis intervention, and recovery-focused care that truly changes lives.
What You'll Do:
Conduct clinical assessments and create personalized treatment plans. Provide direct therapy and crisis intervention in community-based settings. Collaborate with psychiatry, nursing, care coordinators, and community partners. Serve as a trusted liaison for clients navigating hospitals, outpatient services, and housing resources. Document care accurately in our EHR while maintaining compliance and confidentiality. Participate in team meetings, case reviews, and professional development.
What We're Looking For:
Active, unrestricted Colorado licensure as LCSW, LPC, or LMFT. Experience with SPMI populations, especially psychosis or co-occurring conditions. Comfort providing care in community or non-traditional settings. Strong clinical judgment, communication skills, and teamwork orientation. Valid Colorado driver's license (for community visits). Job Summary:
As a Licensed Outpatient Therapist at Diversus Health, you will share our vision for improving behavioral health care in our communities and grow in an environment driven by our core values. In addition to providing quality care, focus in this role is on clinical confidence and further expanding clinical knowledge and expertise as a licensed professional. Essential Functions -
Clinical Care:
Evaluate mental health diagnosis, create, and implement a treatment plan, complete ongoing documentation including further diagnosis, treatment plan reviews, and case notes according to company policy Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community Utilize creativity in interventions to help clients achieve and exceed goals Prepare and submit individual documentation for each session per company guidelines and protocol For Full-Time status clinicians must maintain a minimum of 30 client visits per week Coordinate services with case managers, families, work personnel, medical personnel, other Diversus Health staff, and school staff as needed Attend and participate in all clinical staff meetings and trainings The Environment at Diversus Health is fluid. Roles and responsibilities may be altered to accommodate changing business conditions and objectives. Employees may be asked to perform duties that are outside of the specific work that is listed. This position may require you to work standard hours, as well as flexible hours before and after standard hours, and overtime in excess of 40 hours in a work week.
Knowledge/Skills/Abilities:
Candidates are required to have a master's degree in one of the behavioral sciences or related fields from an accredited college or university Candidates should have clinical licensure (LMFT, LCSW, LPC, PsyD etc.) Required experience with completing DAs, treatment plans and clinical case notes Effective written and verbal communication skills Ability to demonstrate and model stable, appropriate boundaries with clients Ability to complete and submit documentation of services and other documents in a timely manner, ideally by end of end or within 24 hours; concurrent/collaborative documentation is a plus Comfort and familiarity working with a diverse high needs client base Proficient in the use of Office 365 and Electronic Health Record systems Your next career move starts here.
