6/19/2026 About the Role Clarvida is seeking a Care Coordinator / Medical Case Manager to support individuals of all ages who live independently or in assisted community settings and have complex health needs. In this role, you will partner directly with clients to develop Health Action Plans and meet with them monthly in their homes to work toward personalized goals related to recovery, independence, and improved health outcomes. You will serve as a central point of contact and advocate, coordinating care with healthcare providers and community partners. This position promotes skill development, connection to resources, and client empowerment to navigate healthcare systems more independently. Responsibilities Develop Health Action Plans collaboratively with clients Meet with clients monthly in their homes to support goal progression Coordinate care with healthcare providers and multidisciplinary team members Serve as an advocate and primary point of contact for clients Link clients to community resources and support services Promote skill-building toward recovery and improved health status Support clients in navigating and independently accessing healthcare services Maintain accurate and timely documentation in accordance with program standards Requirements Bachelor's degree in a Human Services, Social Services, Nursing, or related field (Social Work, Psychology, Counseling, etc.) Minimum of two years of experience working in nursing, healthcare, behavioral health, or human services (paid or volunteer) Valid Washington Driver's License and active automobile insurance Ability to pass state and federal background checks Compensation & Benefits Full-Time Employees Competitive pay at $20
