More about our team Inpatient Rehabilitation at Kent Hospital , located at 455 Toll Gate Road, Warwick, RI 02886, offers customized patient and family-focused rehabilitation treatment and education that emphasize each patient's unique strengths, abilities and preferences. Rehabilitative care begins with appropriate therapies that lead the patient to the greatest degree of function possible, with individualized therapy goals to prepare patients once again, to enter the world of independent living. Kent Hospital works with partners to provide an uninterrupted treatment path from inpatient rehabilitation to home care and outpatient rehabilitation. How you'll contribute The Case Manager for the Acute Rehabilitation (ARU) unit coordinates and manages patient care to ensure high‑quality outcomes, optimal resource utilization, and compliance with regulatory and external review requirements. This role provides comprehensive assessment, care planning, coordination, and ongoing evaluation of individual patient needs while honoring patient preferences and supporting continuity of care. Serving as a patient advocate, the Case Manager facilitates discharge planning, addresses holistic needs, and communicates effectively with patients, families, and the interdisciplinary team. Through the integration of case management, utilization review, and discharge planning functions, this position enhances patient satisfaction, promotes cost‑effective care, and supports the hospital's mission, vision, and values. What we're looking for Applicants should have Current Registered Nurse (RN) or Social Work licensure (LMSW, LCSW, LICSW) or Healthcare professional licensure as Physical Therapist (PT), Speech Language Pathologist (SLP) or Occupational Therapist (OT). Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC Minimum of two (2) years social work or case management experience in an inpatient setting highly preferred Acute/rehabilitation hospital experience preferred.
