Work Shift Job Description Job Duties:

  • follow the SC Ryan White Part B Medical Case Management Standards to ensure that all points of care are met for each patient enrolled in case management.
Key activities include:
Intake and assessment of service needs Development of a comprehensive, individualized service "action plan." Coordination of services required to implement the plan Client monitoring to assess the efficacy of the plan Periodic re-evaluation and adaptation of the plan over the patient's life. Maintain accurate documentation of services, contact information, and other patient demographics in Provide Enterprise database and Epic. 25%
  • Perform medical case management functions for people receiving HIV care through the MUSC Infectious Disease Clinic, providing adherence counseling to ensure readiness for and adherence to complex
HIV/AIDS
treatments. In addition, the social worker will meet with patients face-to-face in an office or home setting, phone contact, and other forms of communication. 15
  • Maintain accurate documentation of services, contact information, and other patient demographics in Provide Enterprise database and EPIC. Complete mandatory DPH trainings and successfully pass SC RWB Medical Case Management Competency Test within 18 months of hire. In addition, twelve continuing education hours in case management/HIV are required per year. 10
  • Assist patients in applying to state and federal programs such as Medicaid. Refer patients to housing services and other area resources. Coordinate and communicate with medical providers. Assist with treatment adherence and medical education in support of medical directives. 10
  • Provide cultural and literacy-sensitive communication to clients with varied language and literacy barriers. Follow up and support patients toward individual competence and life skills to promote treatment, adherence and retention in medical care. 5

Preferred Experience & Additional Skills:
Master's in social work and South Carolina Social Work License preferred. Medical Case Management experience preferred. Knowledge of policies and procedures relating to establishing and maintaining community social service programs. Knowledge of specialized needs in education and treatment for emotionally and/or environmentally handicapped clients. Ability to establish and maintain effective working relationships with necessary agencies, organizations, and interested public or private groups. Ability to present clear and comprehensive oral and written reports. Ability to identify and effectively utilize resources available in the community to accomplish objectives. Ability to organize and maintain a caseload as necessary. Ability to communicate effectively.
