Job Description

Position Summary The Case Manager is responsible for coordinating client care, supporting treatment planning, and facilitating some psychoeducational and therapeutic groups. This role serves as a key liaison between clients, clinical staff, and external providers, ensuring continuity of care while promoting client engagement, accountability, and progress in treatment. While group facilitation is not required for all candidates, preference will be given to individuals who are qualified and comfortable assisting with group services. Key Responsibilities Coordinate and monitor client services in accordance with individualized treatment plans Maintain regular communication with clients regarding treatment goals, appointments, and aftercare planning Coordinate referrals to community resources, medical providers, housing, employment, legal, and social services as appropriate Support discharge planning and continuity of care, including aftercare and relapse prevention planning Communicate effectively with clinical staff to report client progress, barriers, and concerns Maintain accurate, timely, and compliant documentation in the electronic health record Qualifications Relevant experience in behavioral health, substance use treatment, or case management preferred Experience facilitating groups or willingness to be trained Group facilitation experience or credentials are not required but strongly preferred; candidates qualified to support or lead groups will be considered an asset to the team Strong organizational, communication, and documentation skills Licensure / Certification Certification or licensure such as CM, CADC, LADAC, or equivalent preferred for running group sessions, depending on state requirements (if applicable) Skills & Competencies Ability to build rapport and maintain professional boundaries Strong group facilitation and interpersonal skills (if applicable) Effective time management and ability to manage a caseload Team-oriented mindset with the ability to work independently Proficiency with electronic medical records Physical & Work Requirements Ability to sit, stand, and move throughout the facility as needed\ Ability to facilitate groups and engage with clients for extended periods Ability to respond calmly and professionally in crisis situations
