In this vital role, you will coordinate patient care plans, facilitate communication among healthcare providers, and ensure that patients receive comprehensive support throughout their treatment journey. Your expertise will help improve patient outcomes, streamline medical documentation, and uphold the highest standards of confidentiality and compliance. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced, collaborative environment. Duties Develop and implement individualized care plans by assessing patient needs, medical history, and physiological factors. Manage and maintain accurate medical records, ensuring compliance with HIPAA regulations and confidentiality standards. Review and interpret medical documentation, including medical terminology, anatomy knowledge, and physiology reports to inform care strategies. Facilitate communication between patients, families, healthcare providers, and legal teams to ensure clarity and continuity of care. Conduct patient care activities such as assessments, follow-ups, and support for critical care or ICU patients. Maintain detailed records of all case management activities, including medical records and legal documentation when applicable. Stay current with healthcare regulations, law office procedures related to patient rights, and best practices in medical documentation. Experience Prior experience in severe mental health or psychiatric hospital settings is highly preferred to understand complex patient needs. Mental health experience or hospital background will enhance your ability to manage diverse patient cases effectively. Strong knowledge of medical terminology, anatomy, physiology, and HIPAA compliance is essential for accurate case management. Proven ability to handle sensitive information with discretion while maintaining thorough medical documentation. Experience working with multidisciplinary teams in healthcare environments such as hospitals or nursing facilities is advantageous. Excellent communication skills to liaise effectively among patients, families, healthcare providers, and legal professionals.
