Job Description
The ACT Team BHT is responsible for providing high quality care in the outpatient setting in which they work. The BHT is responsible for utilizing good skills in assessment, treatment planning, case management, and the documentation of services. The BHT is expected to function well as a member of the Clinical Team. Responsible for providing targeted case management services and assessments, service planning and evaluation, linking, advocacy, and crisis intervention to assigned case load. 1. Minimum of High School Diploma, but Associates in Human Services field preferred. 2. Two years of clinical experience in the behavioral health field is strongly preferred. 3. Required documentation must be current and in compliance with Medicaid and licensure standards. 4. Must maintain a favorable rating on performance evaluations. 5. Must have a valid driver's license and a clean driving record.
